
7 Steps to Becoming a Healthy Arizona Worksite

HAWP 101 TRAINING
The first step in applying for the Healthy Arizona Worksite Award is to attend a Healthy Arizona Worksites Program (HAWP) 101 training. The HAWP training has been developed to help organizations learn how to implement evidence-based worksite health initiatives that can improve business and employee health.
If you are new to your worksite and have not attended this training, you will need to complete it before beginning the application process. If you have already completed the HAWP 101 training, you DO NOT have to attend a new training again.
LETTER OF SUPPORT
Obtain a letter of support from organization’s leadership to announce the wellness program and demonstrate commitment. Click here for sample letters of support.

CDC WORKSITE HEALTH SCORECARD ASSESSMENT
The CDC Worksite Health ScoreCard is a tool designed to help employers assess the extent to which they have implemented evidence-based health promotion interventions in their worksites. Every worksite must complete a new CDC Worksite Health Scorecard every application year.

POLICY & CHANGE IMPLEMENTATION
Use your CDC Workplace Health Scorecard results (and other additional assessments) to indicate what employee wellness areas need more policy support to be comprehensive. Then use the Employer Resources section of this site to find template policies and tools to implement change. Can’t find a specific policy template? Contact us for more support.

HAWP AWARD APPLICATION
You’re so close! This is the final step to apply for your Healthy Arizona Worksite Award. This application on average takes 30 minutes to complete and is recommended to be completed by your wellness team lead.
If your worksite has previously received a Copper, Silver, or Gold HAWP Award and wishes to remain at their current level, you are only required to submit a new CDC ScoreCard, NOT a new application.
Worksites that wish to go on to the next level MUST submit a new application.
If you previously received a Platinum Award, you MUST submit a new CDC ScoreCard and a narrative of the healthy community partnership strategy.
