How to apply for

The Healthy Arizona Worksite Award recognizes employers for their efforts to support employee health and wellness. There are four levels of the award: Copper, Silver, Gold and Platinum. Each level of the award requires employers to attend the HAWP 101 training, obtain a letter of leadership support for worksite wellness, complete the CDC Worksite Health Sccorecard, and write a worksite health improvement plan. The higher levels of the award- Silver, Gold, and Platinum- require implementation of additional worksite wellness initiatives. To receive the Platinum level of the Healthy Arizona Worksite Award, employers must be evaluating their initiatives and working on an additional project.

Yes, you do have to submit another scorecard regardless of your application level.

No, you do not have to submit another application if you are applying for an award on the same level as your previous application.

They are notified on a rolling basis but will receive official confirmation after the Awards deadline, which is usually in early May.

All awarded applicants will be notified of the Awards Event date, time, and location.
